• Name, address and phone number of landlord(s) for the past two years
• Name, address and phone number of employer(s) for the past two years
• Copies of pay stubs for the most recent 30-day period
• Social Security income, if applicable
• Copies of W-2 forms for the past two years
• Statements for the last two consecutive months for all personal bank accounts, retirement accounts and investment accounts
• Copies of the last two years' individual income tax returns. Include copies of K-1s, if applicable
• Copy of driver's license and Social Security card
